Position Overview
We are seeking a Family Nurse Practitioner (FNP) to provide high-quality, patient-centered primary care across the lifespan. This role is ideal for a clinician who values flexibility, autonomy, and mission-driven work while practicing in a supportive, collaborative environment.
Key Responsibilities
- Provide comprehensive primary care services, including assessment, diagnosis, and treatment of acute and chronic conditions
- Perform health screenings, preventive care, and wellness visits
- Prescribe medications and manage ongoing treatment plans in accordance with state regulations
- Collaborate with behavioral health providers and care team members to support whole-person care
- Document care accurately in the electronic health record
- Participate in quality improvement and patient-centered care initiatives
Qualifications
- Licensed Family Nurse Practitioner (FNP) in the state of Michigan (or eligible for licensure)
- National board certification (AANP or ANCC)
- Strong clinical judgment and commitment to evidence-based practice
- Experience working with diverse or underserved populations preferred
- Comfort with telehealth platforms is a plus
- Bilingual or multicultural experience welcomed
